I have read all trip policies, including behavior expectations, on the Parent Info & Trip Policies page and have reviewed them with my teen.*

I understand my child will be transported in a personal vehicle and give my permission as part of the Unplugged Outdoor Co. program.*

I authorize Unplugged Outdoors Co. staff to administer basic first aid to my child if needed (such as bandages, cold compresses, or antiseptic). I also give permission for staff to provide standard, over-the-counter medications, such as acetaminophen, ibuprofen, or diphenhydramine, in accordance with label instructions and only when appropriate. I understand that no prescription medications will be given.*

If I cannot be reached in an emergency, I authorize Unplugged Outdoor Co. to seek medical care for my child and to act in their best interest. I understand and I am responsible for any medical costs incurred.*

I understand that outdoor activities carry inherent risks. I agree not to hold Unplugged Outdoor Co, its staff, or affiliates responsible for any injury or illness that may occur, except in cases of gross negligence. I release and hold harmless Unplugged Outdoor Co. from any claims or liability. *
